Fidelity Care Home Health is a family owned and operated business. We take care of all patients. We are located in Houston but service to all the surrounding areas. Fidelity Care is a Medicare certified Home Health Agency that is licensed and Certified by the Texas Department of Aging and Disability Services.

We handle all referrals information. Our Intake coordinator will take all referrals information. We will service

the patient’s needs or we will, through our network partnership locate an agency or service that can. Thereby we save you time and energy. We will contact you of the outcome in a timely manner.

Our mission is to be the premier health care agency, providing quality and efficient home care while utilizing the

best clinical expertise.

We believe that home health care is an important part of continuous health care. We provide Home Health Care in a caring, convenient and cost effective manner. Our personnel utilize the team approach when giving home health care and focus on the individual needs of each patient.

· We strive to have the same staff visiting patients whenever feasible to ensure a consistency in patient’s care.

· To help our patients adjust to new disease process and diagnosis, whereby the patients learn to take care of themselves and

maintain optimal health.

· To meet and exceed the home health care needs of all our patients.

· To provide all levels of home care services in conjunction with Physician, thus allowing the client to remain at home.

· Function as a liaison between patients and available community resources.
