Why does a patient need home health care? ¨We teach patients to take care of themselves with their new and/or existing illness Coordinate services: such as Meals on Wheels, provider services, DME services           How quickly can you see a patient? ¨Our goal is within 48 hours How soon will we receive feedback? ¨Within 48 hours of completion of assessment. Do you refer patients to Physicians? ¨Yes, If we find that a patient has a need for a Doctor. What is IV Therapy? ¨Administering IV medication at home.     ie: antibodies, TPN What type of Wound Care do you handle? ¨Anything from stage one ulcer to stage four ulcers to surgical wounds.  Who needs Home Care? If you were recently hospitalized for surgery or illness. If you are unable to meet your own needs due to health, age or social problems. If you are stressed with taking care of a family member who can no longer care for themselves. If you are homebound and lonely, frightened, or depressed. What are your service areas?
